Scholarships and Financial Aid 
Financial Aid is now processed on-line through FAST; no hard copies of financial aid forms except for the EITC scholarship request form.

- Remember to print out two copies of the "Merit Scholarship Reference Form," one copy for the principal/teacher recommendation, and the other for a pastor/youth pastor at your church.

The following students should submit this form:- Students entering grades K, 6, or 11
- Students coming from outside Pennsylvania
- Students wishing to participate in athletics should also submit a PIAA Physical Form.
School Activities
- Educational Activity Excuse Request
- Before planned trips of one-half day or more, a student should complete this form and have it signed by a parent or guardian. If a student is going to be absent for three days or more, the back of the form must be completed and signed by each of the student's teachers. Forms should be turned in five school days before the trip.

- Behind-the-Wheel Driver Ed
Registration
- LMS works together with Isabel Bentz from A-1 Mumma's Driver Training School to provide behind-the-wheel driver instruction to students. This form, along with a check for the amount listed on the form, should be returned to the LMS office for processing. Mrs. Bentz will then contact the student directly to schedule a convenient time for driving (either during or after school).
