Request for Official Transcripts – Lancaster Campus

- Complete the Parent Permission for Transcript Release form. Select the box below to print.
- Complete the Student Official Transcript Request form. Select the box below and follow the directions at the top of the page.
- Is your university part of the Common App? If the answer is yes, you do not need to complete step #2 above. Your guidance counselor will send your transcript to the college once you’ve completed the application through the Common Application website.

- Please allow 10-15 working days to process your transcript request.
